Here is a complete guide on how to do it:

I've recently written an instructable on using the Cosmos kernel (packaged as the Cosmos "user kit") to create a custom operating system. No hassle, no tricky bits, no cookie cutter features. Just a foundation and an interface. Link:
You can also try SUSE Studio, a webapp that lets you customize what's included in a SUSE distribution.  It may take a day or two to get an account with these guys, but they're worth it.

I knew of a few others that did this, but I'm forgetting the names.  I'll amend this post if I think of another one.
Crap, now I have Phil Collins in my head singing, "Su- se- studio!"

Glad someone else shares my pain...
Slax has a similar web app where you can choose the modules that are included:
That's the big one that I was forgetting.  Thanks!

Question: Why?

What will your new operating system do differently (preferably better) than the existing ones? If you can't answer that, you will probably learn as much or more by learning to work with one of those existing systems.

There's a book that explains how to create a simple OS called Minix.  Google minix and you should find it.
