Very carefully remove the tacks holding the existing upholstery on.

Lay the pieces out flat to measure them and estimate you material needs.

Purchase new material. 

Cut new material, including padding, in the shape of the old material.

Put new material on chair just like old material.

Sit in chair and enjoy.

Depending on the shape of the cushions and accessibility of the frame, it might be advisable to cut the pieces a bit larger than the ones that were removed. Sometimes you need a little extra to hold onto whle you stretch and staple/tack the fabric on. You can always trim the excess after it's all attached securely.
