If you mean an Instructables patch, it's a (virtual) unofficial award: a small graphic and accompanying text which appears on your user page. "Pro" (paying) users can send patches as another way of thanking/applauding other users, based on whatever criteria they consider appropriate. Anyone can receive a patch.
It doesn't give you anything but amusement and possibly bragging rights.
