It looks as though there's not "best answer" So this looks like a poor user interface rather than a bug. The "answered" questions have a best answer selected and the "unanswered" question may indeed have already been answered, just that a "best answer" has not been selected.
That's correct. "Answered" means someone provided a response which the author identified as Best Answer. Unanswered means that the author doesn't like any of the responses (or the author is too lazy to mark one of them Best).
