Most people push off until they have some momentum, and then climb up quickly while the bike continues to roll forward. Tall bikes are really cool to ride on the playa once you get going. Not sure I'd want to ride one around town though. That said, I watched a guy on a tall bike win the "slow bike" race (where the winner is the last person to cross the line while staying in lane and not putting your feet down) at Tempe's Tour de Fat back in 2007. Mad skillz.
