Chest and shoulder pad pieces
Four sets of pads were made from a single foam sleeping pad.  These pads are about $6 at Walmart.

For each set of pads, I began by cutting a rectangular piece of foam 20" by 9". 

A 5 1/2" diameter circle was drawn in the center, and lines were drawn to taper the sides down to 6 inches on either end.

I cut out the middle circle and the tapers on the sides with a utility knife.

I've attached a PDF with the pattern for the shoulder pad piece.  The sides of the wedge portion of each shoulder pad were glued together with hot glue to make each shoulder pad slightly rounded.  I trimmed the square edges with scissors (see photos 2 and 3).
