In the video there is a picture of the template. Make a screenshot, and print as desired measures. Keeping only the aspect ratio.

This is really not very easy to do on YouTube. Also sitting through the entire video to find the crease diagram is not so pleasant either. I've tried watching several of your videos, and while they're thorough and detailed, they're not easy to follow. I'd personally prefer to read the instructions instead of sitting through 20 minutes trying to follow along or find one screenshot. Could you provide cut patterns on a webpage? You'd get a lot more followers if you did.
