Lemonie is almost, but not quite right, I think. Steps without images are still handled the way they always have been -- the thumbnail is "talk to the hand," and there's no image or box or anything else in the step content.
The step you're showing doesn't have an actual image associated with it, but you did include a binary file (a nasty MS Word document :-), as the only attached file. The I'bles server is using its built-in MS-Word icon as the "thumbnail" image, but it of course has no way to "preview" a Word document inline, leaving you with the weird box.
Either do what you are supposed to do, and provide an actual image with that step (sorted so the image comes first, before the Word document), or just live with it.
The site is now picking up that there is no image on steps such as that and dropping in a default gif. I guess this is to make it obvious that a picture has been skipped when I'bles are being composed.
The robot complains sometimes. But now, if you don't have a functional image the page shows the big empty space where one should be. It may also help with some page / pdf formatting?

Ahh I see, I have not found that when there was no image there is a problem though...
