It's a way of another Instructables member congratulating you. Either because you earned it or your just awesome ;)

They are pretty fun to get. Only pros can send then but anyone can receive them.
I only have three, but hey that's some! :-)

Note that you can search for things on Instructables -- that "Google custom search" box at the upper right of the page -- and feeding your question into that should find a fair number of previous Answers to this same question.

Hmm.

Well, for clothing, it's apiece of fabric used to mend a hole

For programming, it's a piece of code used to fix a problem,. usually one that's encountered unexpectedly and needs to be fixed quickly

For Instructables, it's a little picture that subscribed members can send to indicate some form of acknowledgement of another member's conduct, text, or instructable. (the recipient need not be a subscribed member of Instructables)
