Step 1:

Take the right strand, go under the middle, come up and put it in the loop.

Step 2:

Take the left strand, go under the middle, come up to put the strand in the loop as you did with the right strand.

Step 3:

Repeat until you get to the end and it should look something like the picture. To finish it, tie an overhand knot, cut the excess cord, and fuse it.
