This design is based on an instructable by golics. I modified it because I wanted to be able to print pictures, designs or text on my CD cover. I also find a template easier and cleaner than origami techniques. When you make this you will not have creases on the back or anywhere you don't want them.
I have attached two PDF files of the template. The first is the Christmas stripe design pictured here (minus the text) and the second is a blank sheet with the cut and fold lines drawn.
Use as is, or import into your favorite program to add pictures or text.

Step 1: Customize and Print the Template

After you've customized your CD cover, print it on letter sized paper (8.5" x 11"). Center the artwork on your page so less of the pattern will be cut off by your printer's margins.

Make sure your printer does NOT shrink the image to fit onto the page. You must print this at 100% size. Most likely you will get a warning telling you that part of the image will be cut off. This is OK, it's part of the design (the margin will vary in size depending on your printer, so it might not look exactly like my version).

Step 2: Cut

Cut solid lines (on the plain template) or where the pattern changes on the Christmas stripes. On the Christmas template you will also see very fine red lines as a cutting guide where the pattern does not change. Don't forget to cut the notch half way across the two opposite corners!

Step 3: Score

Turn the page over, and with a ball point pen and a ruler firmly draw lines from one cut to the next to draw your central octagon. This will make folding much easier.

If you're using the plain template just use the ruler to trace along the dotted lines.

Step 4: Fold

Start by folding the two corners which aren't notched. The are no guides on the Christmas template for this, it's easy enough to eyeball. There is about a centimeter between the corner of my fold and the cuts on either side.
Starting next to one of the notched corners, fold all the flaps (skipping the second notched corner). Tuck the corner of your last flap under the first flap you folded. Fold the last two corners. No need to notch them now, because you're just going to open them all up again. In this case, "wear and tear" is literally what you want to avoid.

Step 5: Insert Your CD

Insert your CD, fold the case again following the instructions on the last step, but this time carefully slip the notches into each other and press it down flat. It's a snug fit, but if you've cut and folded accurately it should work.
